Toronto Monthly Meeting COVID-19 Visitor Waiver

Attenders of the in-person component of Meeting for Worship must sign a COVID Waiver, which can be submitted electronically here, if you'd like to make the process more efficient.  It can also be mailed to Friends House or delivered in person. It only needs to be submitted once.
