NOTE: In addition to the Release form, an Organizer and Participant waiver, which will include contact tracing information, will need to be signed by each participant of the booked event(s) before they enter Friends House. A copy of the Organizer and Participant waiver will be sent to the event organizer, with instructions for distribution to event participants, once the Release has been processed.
